To order your personalized SurgePaysTM Visa® Prepaid Card, you will be required to provide your name, address, date of birth and other information that will allow us to identify you. Federal law requires all financial institutions and their third-party affiliates to obtain, verify and record information that identifies each person who opens a Card Account. The SurgePaysTM Visa® Prepaid Card is currently not offered to residents of Connecticut and Vermont.
The SurgePaysTM Visa® Prepaid Card is issued by Central Bank of Kansas City, Member FDIC, pursuant to a license from Visa U.S.A., Inc. Certain fees, terms and conditions are associated with the approval, maintenance, and use of the Card. You should consult your Cardholder Agreement and Fee Schedule.
